Baseline data from November 2025, demonstrates a fall rate of 16.03 falls per 1,000 resident days (25 falls/1,559 resident days).

Following implementation of the Monarch Stop the Drop fall prevention system in December 2025, the facility's fall rate decreased to 6.81 falls per 1,000 resident day (11 falls/1,616 resident days), representing a 57.5% reduction from baseline.

In January 2026, the fall rate further decreased to 5.06 falls per 1,000 resident days (8 falls/1,581 resident days), reflecting a sustained 68.4% reduction from baseline.

As of mid-February 2026, the facility has experienced 7 falls resulting in a fall rate of 4.66 falls per 1,000 resident days. This reflects a 70.9% reduction from the November baseline following implementation of the Stop the Drop system.
