Staff are trained to notice and report early warning signs and near-miss events, including unsafe transfers, unassisted ambulation, environmental hazards, and changes in behavior that signal increased risk.

Visual mobility tags and alerts create clear, consistent communication across all shifts and disciplines, ensuring everyone understands a resident’s current level of risk.

Near-miss information is reviewed by leadership through QAPI to identify patterns, trends, and residents who may require additional support.

This allows care plans, supervision, and interventions to be adjusted in real time - before an injury, investigation or citation occurs.
